Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinic setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs and goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to coordinate patient care
• Monitor and track patient progress, adjusting treatment plans as appropriate

Required Qualifications

• Active Physical Therapist license in the state of Arizona (permanent license required)
• Prior experience in a physical therapy clinical setting
• Ability to meet credentialing and compliance requirements prior to assignment start
• Must maintain licensure in good standing throughout the duration of the assignment

Preferred Experience / Skills

• Experience in an outpatient physical therapy environment
• Familiarity with electronic medical records and clinical documentation systems
