Key Responsibilities

• Assess, plan, implement, and coordinate individualized plans of care for assigned residents, making updates as clinical needs change.
• Administer medications and treatments to assigned residents in accordance with physicians' orders, ensuring timely notification of physicians as required.
• Observe, document, and report residents' conditions to appropriate staff; respond promptly to resident call lights.
• Maintain residents' self-esteem, privacy, and confidentiality while promoting independence and choice consistent with care plans.
• Utilize the electronic medical record (EMR) system accurately and employ proper transfer equipment and handling techniques to ensure resident safety.
• Assist with resident and family education and demonstrate knowledge of department-specific safety, fire, and disaster plans.

Required Qualifications

• Graduate of an accredited school of nursing.
• Active Registered Nurse (RN) licensure in the state of practice.
• Current Basic Life Support (BLS) certification.
• Knowledge of basic nursing skills and familiarity with therapeutic effects and side effects of administered medications.
• Ability to effectively communicate in English, both orally and in writing.
• Ability to lift up to 35 lbs. and perform the physical demands required to carry out essential job functions.

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility (LTC/SNF) setting.
• Familiarity with electronic medical record (EMR) systems used in skilled nursing environments.
