Contract | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide occupational therapy services to home health patients in Hood River, OR and surrounding areas, visiting up to 6 patients per day
- Complete OASIS documentation accurately and in a timely manner using EPIC Remote Client and Rover mobile application
- Deliver skilled occupational therapy interventions for patients with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke conditions
- Support patients with activities of daily living including functional activities related to acute and chronic disorders
- Manage daily schedule independently with reliable personal transportation; occasional service radius of up to approximately 60 miles per day
Required Qualifications
- Minimum 1 year of skilled home health occupational therapy experience
- Active Oregon (OR) and Washington (WA) OT licensure required (pending license accepted)
- OASIS documentation experience required
- Basic Cardiac Life Support (BCLS) certification required
- Reliable personal transportation required
Preferred Experience / Skills
- Experience with National Patient Safety Goals and Core Measures
- Background in home health billing practices
- Proficiency with OASIS documentation workflows and home health charting platforms
- Experience addressing acute and chronic disorder functional activity needs
