Contract | Rotating 8-Hour Shifts | Onsite | Includes Weekends and Holidays | Varied Schedule – Flexibility Required | No Guaranteed Hours or Shift Preference

Key Responsibilities

• Perform patient assessments and administer medications and treatments as directed
• Notify MD/NP of pertinent patient information and changes in condition
• Assist with admissions, discharges, and activities of daily living (ADLs)
• Provide specialized care including Gastrostomy Tube care, Urinary Catheter care, and Nebulized Treatments
• Deliver advanced clinical care including Wound Vac care, CPAP care, and Tracheostomy Care
• Perform all other duties as assigned within the long-term care setting

Required Qualifications

• Active LPN or LVN license in good standing
• Experience in a Long-Term Care or Skilled Nursing Facility (LTC/SNF) setting
• Ability to work a rotating schedule including weekends and holidays
• Proof of current flu vaccination required prior to contract start date

Preferred Experience / Skills

• Experience with Tracheostomy Care, Wound Vac, and CPAP management
• Familiarity with admissions and discharge processes in a long-term care environment
