Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite | May include occasional weekend shifts
Key Responsibilities
- Provide skilled home health nursing care to patients in a 1:1 setting, visiting up to 6 patients per day
- Complete and maintain accurate clinical documentation using EPIC and OASIS
- Meet productivity point expectations in accordance with agency standards (30 points standard)
- Manage a patient caseload within an assigned geographic service area, requiring independent travel between patient homes
- Coordinate care and communicate effectively with the broader care team to support patient outcomes
Required Qualifications
- Active Registered Nurse (RN) license (pending license considered)
- Minimum 1 year of skilled home health nursing experience
- OASIS experience required
- Basic Cardiac Life Support (BCLS) certification required
- COVID-19 primary vaccination series plus at least one booster required
- Reliable personal transportation required; occasional travel radius may exceed 50 miles
Preferred Experience / Skills
- Experience with EPIC electronic health records system
- Familiarity with home health productivity point systems and expectations
