Key Responsibilities

• Perform assessments and reassessments per policy and procedure or as warranted by patient condition, demonstrating the ability to make sound independent clinical decisions
• Initiate, prioritize, and implement individualized plans of care; review and revise plans as indicated in collaboration with patients, families, and interdisciplinary teams
• Monitor and document patient responses to clinical interventions
• Facilitate, coordinate, provide, and evaluate patient and family education specific to care, treatment, and services during the stay and at discharge
• Delegate tasks to licensed and non-licensed personnel; serve as a mentor and preceptor while promoting teamwork and a collaborative care environment
• Participate in quality, safety, and service process improvement efforts; comply with regulatory standards and contribute to the Practice Governance model

Required Qualifications

• Active Registered Nurse (RN) licensure
• Nursing Diploma or Associate's Degree in Nursing (ASN)
• Minimum of two (2) years of acute care RN experience
• Current BLS certification
• Current NIHSS certification
