Key Responsibilities

• Provide direct social work services to patients and residents in a specialty care setting
• Assess patient and family needs and develop appropriate care plans in coordination with the interdisciplinary team
• Facilitate discharge planning and coordinate community resources and support services
• Document social work assessments, interventions, and progress in accordance with facility and regulatory standards
• Serve as an advocate for patients and families throughout the care continuum

Required Qualifications

• Bachelor's degree in Social Work from an accredited program
• Active and applicable Minnesota Social Work license required
• Ability to work a Sunday through Saturday work week schedule
• Availability to fulfill guaranteed weekly hours (36 hours for 12-hour shifts or 40 hours for 8–10 hour shifts)

Preferred Experience / Skills

• Prior experience in a specialty care, long-term care, or skilled nursing facility environment
• Familiarity with care coordination and discharge planning processes
