Licensed Practical Nurse – Long Term Care

Contract | 13–16 Week Assignment | Mid Shift | Onsite
Key Responsibilities
- Provide direct patient care in a long-term care / skilled nursing facility setting
- Work a rotating schedule that includes a minimum of every other weekend, adjusted to meet unit and patient care needs
- Cover CNA duties as needed in addition to standard LPN responsibilities
- Participate in bi-weekly Monday orientations as required
- Work 2 out of 4 designated holidays: Thanksgiving, Christmas, Christmas Eve, and New Year's Day
- Maintain complete and accurate work history documentation including bed count, EMR, and facility type
Required Qualifications
- Active LPN license in New York State, or documented proof of application with applied date
- Minimum of 1 year of LPN experience
- Long-term care (LTC) experience required
- American Heart Association (AHA) BLS certification required
- Reliable personal or rental transportation required; public transportation is not accessible in this area
- All employment gaps exceeding 90 days must be explained
Preferred Experience / Skills
- Additional certifications beyond AHA BLS are a plus
- Familiarity with electronic medical records (EMR) systems used in long-term care settings
