• What You’ll Be Doing:
  • Oversee clinical operations of the home health agency ensuring high-quality client care
  • Conduct initial and ongoing client assessments and document in WellSky
  • Develop, monitor, and update individualized care plans and communicate changes to caregivers
  • Supervise approximately 40–45 active caregivers, ensuring quality care and compliance
  • Conduct client follow-up visits approximately every 3 months or as needed
  • Support caregiver onboarding and competency sign-offs
  • Respond promptly to client/family concerns and caregiver performance issues
  • Maintain accurate nursing documentation and regulatory reports
  • Serve as the agency’s nursing lead during Joint Commission surveys
  • Provide clinical guidance and support to the team, including participation in rotating on-call schedules

What We’re Looking For:

• Active, unrestricted RN license
• Nursing degree/college graduate
• Current CPR certification
• Proven experience in home health, geriatric, LTC, or community nursing
• Working knowledge of AHCA and Joint Commission standards
• Ability to confidently manage clinical documentation and compliance
• Strong communication, leadership, and problem-solving skills
• Ability to work independently and manage multiple priorities
• Willingness to participate in rotating on-call coverage
