Schedule / Job Type

Contract | Days | Expected 5 days per week, Monday–Friday 8:00 AM–4:30 PM, with rotating weekend coverage and evening on-call rotation

Key Responsibilities

• Provide skilled nursing care to Home Health and Hospice patients with diagnoses including CHF, Diabetes, Renal Disease, and Wound Care, as well as IV therapy and management of drains and other devices
• Conduct Start of Care (SOC) assessments, Re-certifications, and Resumptions of Care; expected caseload of approximately 6 points per day, including an admission plus 3–4 patient visits
• Complete OASIS documentation with proficiency in accordance with Home Health standards
• Participate in the on-call rotation, typically no more than one night of call per week, and rotating weekend coverage approximately every 4th or 5th weekend
• May be required to see Hospice patients during on-call shifts; training on Hospice protocols will be provided as needed
• Adhere to excellent infection control practices within the home environment

Required Qualifications

• Active RN (Registered Nurse) license valid in the state of Michigan
• BLS certification required
• Recent Home Health experience within the past 2 years
• Proficiency in OASIS documentation
• Demonstrated wound care experience and strong infection control practices in the home setting
• Candidates living within 80 miles of the facility are considered local and may be subject to adjusted rate eligibility

Preferred Experience / Skills

• Experience with IV therapy and management of medical drains and devices
• Familiarity with Hospice care protocols
• Experience working with diverse patient populations including CHF, Diabetes, and Renal Disease
