Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:00 PM) | Onsite | Occasional Weekend Requirement
Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting under the supervision of a licensed Physical Therapist
- Manage a caseload of approximately 6–10 patients
- Accurately document patient care and progress using the Homecare Homebase charting system
- Complete required orientation modules upon onboarding
- Participate in occasional weekend coverage as needed
Required Qualifications
- Active Tennessee PTA license or valid compact license – must be in hand at time of submission; no pending licenses accepted
- Minimum of 6 months of recent or current home health experience required
- Home health travel experience required
- First-time travelers will not be considered for this role
- Proficiency or ability to work with Homecare Homebase electronic charting system
Preferred Experience / Skills
- Familiarity with Cerner documentation systems
- Experience with flexible or block scheduling arrangements
