Key Responsibilities

• Deliver skilled physical therapy services to adult and geriatric patients in their homes across a rural, geographically diverse service area
• Collaborate with supervising physical therapists and an interdisciplinary care team to implement individualized treatment plans
• Support patient recovery from illness, injury, surgery, and chronic disease while promoting independence and preventing hospitalizations
• Complete approximately 6 patient visits per day across the designated service area
• Document patient progress and treatment accurately and in a timely manner

Required Qualifications

• Active Virginia PTA license required to be in hand at time of submittal, or valid compact license with proof provided
• BLS certification through the American Heart Association (AHA) required
• Specialty experience within the last year required
• Valid driver's license and reliable transportation required for home visits throughout the service area
• Locals are accepted
• SSN required at time of submittal

Preferred Experience / Skills

• Prior home health experience, particularly in a rural setting
• Experience working with geriatric populations and patients managing chronic disease
• Familiarity with electronic medical records and clinical documentation systems
