Key Responsibilities

• Perform and document comprehensive patient assessments, including identification of risk factors, presenting complaints, and objective/subjective data, in accordance with RN scope of practice and hospital policies
• Develop individualized care plans in coordination with patients, families, and the inter-professional team, incorporating cultural, age-specific, and developmental considerations
• Implement patient care therapies and procedures with clinical autonomy; recognize, respond to, and communicate changes in patient condition appropriately
• Facilitate admission, transfer, and discharge activities, including execution of discharge orders, discharge teaching, and coordination with Case Management
• Delegate tasks appropriately and monitor accuracy and completeness of delegated care provided by other team members
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification required
• Associate Degree in Nursing (ADN or ASN) required
• Ability to communicate and collaborate effectively with all members of the care team
• Proficiency with or ability to be trained on relevant computer systems and clinical applications

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting
• Bachelor of Science in Nursing (BSN)
