Contract | Days | 5x8-Hour Shifts (8:00–16:30) | Onsite
Key Responsibilities
- Provide skilled home health nursing care to patients in the West Portland, OR area and surrounding communities
- Visit up to 6 patients per day, with scheduling designed to minimize drive and windshield time; occasional maximum radius of approximately 60 miles per day
- Complete OASIS assessments as required for home health documentation and compliance
- Chart using EPIC Remote Client for Home Health (laptop) and Rover phone application
- Maintain professional business casual attire; scrubs are permitted but not required
- Provide reliable, independent transportation to patient locations throughout the service area
Required Qualifications
- Active Oregon RN license (compact license must be in hand at time of submission)
- Minimum 1 year of skilled home health nursing experience
- Basic Cardiac Life Support (BCLS) certification required
- OASIS experience required
- COVID-19 primary vaccination series plus one booster required
- Must have reliable personal transportation
- Locals are welcome; candidates within a 50–75 mile radius of the facility are considered local and accepted
Preferred Experience / Skills
- Experience with EPIC Remote Client for Home Health and/or Rover application
- Ability to manage a varied daily patient schedule across a geographic service area efficiently
- Strong time management and independent clinical decision-making skills in a home-based care environment
