Contract | Full-Time | Days, Monday–Friday 8:00 AM – 4:30 PM | Onsite
Key Responsibilities
- Triage emergent situations independently, applying advanced nursing knowledge and skills to manage complex clinical scenarios.
- Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
- Utilize care plans independently to deliver clinic-based and home-based direct nursing care.
- Administer treatments, procedures, and medications as prescribed by the PCP.
- Conduct regular home visits to provide care including wound care, lab draws, nurse assessments, and other nursing tasks as ordered.
- Collaborate with both licensed and non-licensed personnel to support participant care.
Required Qualifications
- Active Registered Nurse (RN) license; nursing diploma or degree from an accredited school of nursing or university required.
- Associate's degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of one (1) year of experience working with frail, elderly, or long-term care populations.
- Current CPR certification (AHA) required or willingness to obtain upon hire.
- Must possess a valid Michigan driver's license, reliable transportation, and current auto insurance.
- Must be 18 years of age or older and fully vaccinated (or willing to be vaccinated) against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
Preferred Experience / Skills
- Minimum of one (1) year of home care experience.
- Minimum of one (1) year of wound care experience.
- Demonstrated ability to work independently in advanced clinical situations with minimal supervision.
