Licensed Practical Nurse (LPN) – Long-Term Care, PM Shift
Location
New Lisbon, WI | Skilled Nursing Facility
Schedule / Job Type
Contract | Full-Time | PM Shift (2:00 PM – 10:00 PM) | Every Other Weekend
Key Responsibilities
- Administer medications to residents following the 5 Rights of medication administration
- Report any changes in resident condition to the charge nurse and update physicians and family members as needed
- Report any allegations of abuse, neglect, or misappropriation to appropriate leadership
- Assist CNAs with answering call lights and other unit responsibilities
- Perform additional duties as requested by facility leadership
Required Qualifications
- Must hold and maintain an active Wisconsin LPN license
- Current CPR/BLS certification required
- Exceptional accuracy and attention to detail
Preferred Experience / Skills
- 1 or more years of related LPN work experience
- Prior experience in a long-term care or skilled nursing facility setting
