Key Responsibilities

• Provide physical therapy services to home health patients in accordance with plan of care
• Complete and maintain OASIS assessments as required for home health documentation
• Chart and document patient progress accurately and in a timely manner
• Travel to patient locations within the assigned service area; mileage reimbursed at IRS rate
• Float to other locations as needed based on operational needs

Required Qualifications

• Active Physical Therapist license; Virginia license required to be in hand at submittal or valid compact license with proof provided
• Home health experience required
• OASIS experience required
• Specialty experience within the last year required
• SSN required at time of submittal
• One supervisory reference required at time of submittal
• Local candidates are welcome and accepted at the same rate

Preferred Experience / Skills

• Familiarity with travel assignment compliance expectations and scheduling flexibility
• Experience managing patient documentation efficiently in a field-based setting
