Schedule / Job Type
Contract | Days | Onsite | Work week: Sunday through Saturday. Expected 36 hours/week for 12-hour shifts, or 40 hours/week for 8–10 hour shifts.
Key Responsibilities
- Provide hospice palliative care to assigned clients and families in accordance with individualized plans of care, demonstrating knowledge of hospice philosophy, scope of services, and continuity of care
- Perform initial and ongoing client assessments, participate in the creation and implementation of the interdisciplinary plan of care, and evaluate clients' needs for additional services
- Coordinate with the interdisciplinary team, process physician orders, and involve clients and families in care planning decisions
- Complete required documentation within appropriate timeframes and participate in quality assurance audits as assigned
- Recognize diverse cultural attitudes toward death and dying; identify stages of the grieving process and the impact of grief on clients and families
- Participate in agency in-services, educational opportunities, team meetings, and committees
Required Qualifications
- Graduate of an accredited school of nursing
- Active RN license for the state of practice required; must hold or be able to obtain a Minnesota nursing license or temporary MN license to support cases in Minnesota
- CPR certification through the American Heart Association (AHA) required
- Must be at least 18 years of age
- Must live within 80 miles of the facility to be eligible for local placement consideration
Preferred Experience / Skills
- Prior experience in hospice or palliative care nursing
- Ability to work independently with minimal supervision while knowing when to seek guidance
- Familiarity with advance directives and interdisciplinary care coordination
