Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaint in accordance with RN scope of practice.
• Develop individualized care plans and determine desired outcomes in coordination with patients, families, and the inter-professional team.
• Implement patient care therapies and procedures with clinical autonomy within scope of practice; recognize, respond to, and communicate changes in patient condition.
• Facilitate admission, transfer, and discharge activities including timely execution of discharge orders, discharge teaching, and coordination with Case Management.
• Delegate tasks appropriately and monitor accuracy and completeness of patient care provided by other team members.
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the applicable state.
• Basic Life Support (BLS) certification required.
• Associate Degree in Nursing (ADN or ASN) required.
• Ability to communicate and collaborate effectively with all members of the care team.
• Proficiency with or ability to be trained on relevant computer systems and applications.

Preferred Experience / Skills

• Previous clinical nursing experience in a progressive care, step-down, or similar acute care setting.
• Bachelor of Science in Nursing (BSN).
