Contract | 13-Week Assignment | Full-Time | Onsite | Evening Shift | 8-Hour Shifts, 5 Days/Week | Expected 40 Hours/Week
Key Responsibilities
- Provide direct patient care to residents in a skilled nursing facility setting
- Administer medications and treatments as prescribed, in accordance with facility policies and state regulations
- Monitor and document patient conditions, reporting changes to the supervising RN or physician
- Assist with wound care, catheter care, and other clinical procedures as directed
- Collaborate with interdisciplinary care teams to support individualized resident care plans
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of New York
- Experience working in a long-term care or skilled nursing facility environment
- Current BLS/CPR certification
- Ability to work evening shifts on a consistent weekly schedule
Preferred Experience / Skills
- Prior travel or contract nursing experience
- Familiarity with electronic health record (EHR) systems used in SNF settings
- Strong communication and documentation skills in a team-based care environment
