Key Responsibilities

• Provide direct patient care in a long-term care/skilled nursing facility setting
• Administer medications and treatments as directed by the care plan and supervising clinicians
• Monitor and document patient conditions, reporting changes to appropriate clinical staff
• Collaborate with interdisciplinary care teams to support resident health and well-being
• Maintain valid state LPN licensure and comply with all facility credentialing requirements
• Adhere to all facility policies, including identification and badge requirements while on site

Required Qualifications

• Active Vermont LPN license in good standing
• Every weekend availability is required for this assignment
• All requested time off must be submitted and approved prior to contract finalization
• Personal reliable transportation is required; public transportation in this area is limited
• Must present a valid state or federal issued photo ID at the facility and wear a photo badge at all times
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer
• Local candidates only

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records and clinical documentation systems
