Key Responsibilities

• Provide physical therapy services to adult and geriatric patients across a defined geographic territory covering the Dayton, Brookville, Eaton, and West Alexandria areas
• Meet weekly productivity expectations of approximately 7 visits per day or 35 visits per week
• Document patient care accurately using the Home Care Home Base EMR system
• Deliver patient-centered care in a home health setting, adapting to varied home environments
• Maintain compliance with all clinical and regulatory requirements throughout the duration of the contract

Required Qualifications

• Active state Physical Therapist (PT) license in Ohio
• CPR certification required
• Minimum of 1 year of physical therapy experience
• Must live more than 50 miles from the facility to be eligible — local travelers are not accepted

Preferred Experience / Skills

• Prior experience working with adult and geriatric patient populations
• Familiarity with Home Care Home Base EMR (training available for candidates without prior experience with this system)
• Previous home health experience preferred
