Key Responsibilities

• Assess, plan, and implement nursing care for residents within a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing
• Administer medications in accordance with the eight (8) medication rights to minimize the potential for medication errors
• Document changes in residents' conditions, nursing care, and care plans accurately and in a timely manner; notify the physician of significant changes and follow through until appropriate action is taken
• Supervise CNAs and supportive staff to ensure continuity of care across shifts, including participation in walking rounds and shift reporting
• Perform and interpret waived/point-of-care testing per applicable policies and procedures, and take appropriate action on results
• Function as a resident advocate by protecting residents' rights and addressing their psychosocial, cultural, ethnic, and spiritual needs with respect and dignity

Required Qualifications

• Graduate of an accredited LVN program
• Current California Licensed Vocational Nurse (LVN) license required
• Current BLS certification required
• Skilled Nursing Facility (SNF) experience required; Sub-Acute experience will not be accepted in lieu of SNF experience
• Ability to read and communicate effectively in English
• Weekend availability is required

Preferred Experience / Skills

• Electronic Health Record (EHR) experience preferred
• Basic computer knowledge
• Proficiency in additional languages is a plus
