Contract | Days | 8:00 AM – 4:30 PM with Rotating Weekends

Key Responsibilities

• Perform comprehensive case management assessments for hospitalized patients to support care coordination and discharge planning
• Collaborate with interdisciplinary teams to develop and implement individualized patient care plans
• Monitor patient progress and facilitate transitions of care across the continuum
• Coordinate with community resources, payers, and post-acute providers to ensure appropriate and timely discharge
• Document case management activities accurately and in accordance with facility and regulatory standards

Required Qualifications

• Active Registered Nurse (RN) license in the state of North Carolina or a compact state license
• Experience in hospital-based case management or utilization review
• Strong knowledge of discharge planning, care coordination, and payer requirements
• Ability to work rotating weekends as part of the scheduled assignment

Preferred Experience / Skills

• Prior experience in an acute care or large hospital setting
• Familiarity with InterQual or Milliman care guidelines
• Case Management certification (ACM or CCM) preferred
