We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in local Long-Term Care and Senior Living facilities. As an LVN/LPN, you will play a vital role in providing high-quality patient care and supporting the well-being of our residents.

Key Responsibilities:

• Assess, plan, implement, and evaluate patient care according to facility policies and procedures • Administer medications, treatments, and procedures as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic patient care tasks • Collaborate with interdisciplinary teams to develop and implement individualized care plans • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date patient records and reports • Participate in quality improvement initiatives and contribute to a culture of safety and excellence

Requirements:
