• Full-time, Days shift, Monday through Friday, weekends rotation

Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility.
• Assist patients with recovery from illness or injury through therapeutic interventions.
• Collaborate with multidisciplinary teams to develop and implement individualized treatment plans.
• Document patient progress and modify treatments as needed.
• Support post-hospital, short-term rehabilitation, long-term, and respite care programs.
• Maintain compliance with facility and regulatory standards.

Required Qualifications

• Valid licensure as a Physical Therapist.
• Must have weekend availability.
• Experience in a skilled nursing or rehabilitation setting preferred.
• Ability to work collaboratively with a healthcare team.
• Strong communication and documentation skills.

Preferred Experience / Skills

• Prior experience in a post-acute or long-term care environment.
• Knowledge of Medicare, Medicaid, and private insurance billing processes.
