Job Title
• Registered Nurse - Home Health

Schedule / Job Type
• Full-time, Day shifts, 5 days per week, 8-hour shifts

Key Responsibilities
• Provide skilled home health nursing services to patients, including wound care, medication teaching, and vital sign monitoring
• Assess patient conditions and communicate status changes to the RN/MD
• Manage pain assessment and medication refill requests via pharmacy communication
• Document patient care using EPIC or equivalent charting systems, ensuring compliance with infection control protocols
• Educate patients and families on treatments and health management plans
• Travel to patient homes, reimbursed at IRS mileage rates, with possible floating based on facility needs

Required Qualifications
• Minimum of 1 year of experience in skilled home health care, including wound care and patient teaching
• Valid driver’s license and clean driving record
• BLS certification
• Must possess or be eligible for VA or compact nursing license
• Ability to provide proof of licenses at submittal
• EPIC charting experience in acute care settings under Riverside Health system

Preferred Experience / Skills
• Experience with infection control protocols
• Prior certification or training in NIH and stroke education for acute care RN roles
• Prior employment within Riverside Health systems or similar healthcare providers
