Key Responsibilities
- Provide education and case management to patients with diabetes, prediabetes, and gestational diabetes.
- Maintain diabetes registries and ensure patients receive recommended annual exams, laboratory tests, immunizations, and education according to IHS Standards of Care.
- Collaborate with the SEARHC Diabetes team to coordinate primary and secondary prevention goals for patients.
- Facilitate education and training on IHS Standards of Care for medical providers, dietitians, nurses, and related personnel.
- Support and model the SEARHC Seven Standards of Excellence.
- Answer inquiries from staff, visitors, and patients in a friendly manner and relay messages appropriately.
Required Qualifications
- Valid, current RN license in the state of Alaska.
- Certification as a Diabetic Educator or eligibility to apply within 2 years.
- Clinical competency required within 3 months of hire and every three years.
Preferred Experience / Skills
- At least 3 years of general nursing or dietetic experience.
- Previous experience in diabetic care and management preferred.
- Knowledge of diabetes case management, health promotion, and patient education.
- Ability to work independently, lead group programs, and engage patients in self-management activities.
- Strong communication skills and cultural competency to work with Alaska Native populations.
