Key Responsibilities

• Coordinate and manage care for patients within the nursing department, including high-risk and complex cases.
• Collaborate with multidisciplinary teams to establish and implement individualized medical goals to improve patient outcomes.
• Facilitate care conferences, track referrals, and ensure timely completion of diagnostics and follow-up.
• Provide patient education, outreach, and support for acute and chronic health needs, including postpartum and OB care management.
• Support specialty clinics by managing referrals, assisting during procedures, and coordinating clinic logistics.
• Communicate effectively with patients, providers, and external facilities to ensure seamless care delivery.

Required Qualifications

• Current and unrestricted Registered Nurse license in Alaska or eligible for licensure.
• Graduate of an accredited nursing program.
• Basic Life Support (BLS) certification.
• Minimum of 1 year of nursing experience, with at least 6 months in a clinical outpatient setting or case management.
• Proficiency in assessment, documentation, and patient education.
• Ability to work independently and efficiently in a fast-paced environment.

Preferred Experience / Skills

• Experience in case management or outpatient care coordination.
• Knowledge of tribal health services and related healthcare organizations in Alaska.
• National Case Management certification.
• Strong communication and problem-solving skills.
