Schedule / Job Type:
Full-time, Day shifts (Monday-Friday, 8:00 AM - 5:00 PM)

Key Responsibilities:

• Provide occupational therapy services in the East region, including Baytown and surrounding areas.
• Conduct client assessments, develop treatment plans, and document progress using Wellsky charting software.
• Call in for all missed visits and communicate with patients regarding evaluation frequency or contact issues.
• Follow professional dress code, including clean scrubs and closed-toe shoes; minimal facial jewelry allowed.
• Ensure visits meet point system requirements, including documentation and visit points, to guarantee weekly hours.
• Manage scheduled visits efficiently, accounting for drive time and mileage reimbursement consistent with GSA standards.

Required Qualifications:

• Valid occupational therapy license with a minimum of 2 years of recent licensed OT experience (excluding private duty and no new grads).
• Current BLS/First Aid certification.
• Ability to call in for missed visits and determine patient contact status.
• Reliable transportation and ability to manage GSA max mileage parameters.
• Professional appearance with proper scrubs and closed-toe shoes.

Preferred Experience / Skills:

• Experience in home health settings.
• Familiarity with Wellsky or similar electronic charting software.
• Ability to work independently within a multidisciplinary team.
