Key Responsibilities

• Provide skilled nursing care to patients in a long-term care or skilled nursing facility.
• Administer medications and treatments accurately following physician orders.
• Monitor patient health status and communicate progress or concerns to the healthcare team.
• Document patient care activities in accordance with facility policies.
• Collaborate with multidisciplinary team to ensure quality patient outcomes.

Required Qualifications

• Valid LVN or LPN license in the state of Wisconsin.
• Minimum of 1 year experience in LTC/SNF settings preferred.
• Basic life support (BLS) certification.
• Ability to work evening shifts and follow clinical protocols.
• Strong communication and patient care skills.

Preferred Experience / Skills

• Previous experience in a skilled nursing or long-term care facility.
• Familiarity with electronic health records (EHR) systems.

Additional Details
Start Date: ASAP or 4/27/2026, pending compliance
Shift: 5x8 hours, evenings (2:45 PM - 11:15 PM)
Guaranteed Hours: 40 hours per week
Work Schedule: Sunday through Saturday (facility operates weekly)
Contract Length: 13 weeks
