Key Responsibilities
• Provide skilled physical therapy services to home health patients in Medford and surrounding areas.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress and treatment outcomes using EPIC/Oasis systems.
• Collaborate with healthcare team members to ensure optimal patient care.
• Minimize drive time through effective patient grouping, with occasional travel within a 50+ mile radius.
Required Qualifications
• Valid state licensed Physical Therapist license.
• BCLS certification.
• At least 1 year of skilled home health experience.
• Own reliable transportation.
• COVID-19 vaccination series plus one booster.
• Experience with Oasis documentation.
Preferred Experience / Skills
• Prior experience working in home health settings.
• Ability to adapt to different patient needs and settings.

• Submit a minimum of two supervisory references covering at least one year of employment within the last three years in a similar setting.
Travel & Additional Notes
• Travelers are expected to float between facilities within the state; refusal to float may result in assignment termination or forfeiture of guaranteed hours.
• The facility provides free parking.
• The facility requires primary COVID vaccination and one booster.
• No weekend shifts or on-call required.
• Guaranteed hours: the facility can cancel up to one shift per pay period.
