Key Responsibilities

  • Provide skilled physical therapy services to home health patients in compliance with care plans.
  • Conduct assessments, develop treatment plans, and document patient progress accurately.
  • Ensure adherence to all regulatory standards, including OSHA and patient safety guidelines.
  • Collaborate with the healthcare team to optimize patient outcomes.
  • Travel within assigned geographic area as required, potentially floating based on facility needs.
  • Maintain all required certifications and licensure in good standing.

Required Qualifications

  • Minimum of 1 year of experience in home health physical therapy.
  • Active and valid VA state license or proof of compact license.
  • Proficiency with Oasis documentation and charting.
  • Certification in BLS, ACLS, and PALS through the American Heart Association (AHA).
  • Must have EPIC charting experience for acute care hospitals under Riverside Health.
  • NIH and stroke education completed for applicable facilities.

Preferred Experience / Skills

  • Recent specialty experience within the last year.
  • Supervisory or leadership experience is a plus.
