Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Schedule / Job Type
Full-time, mids shift (8.5 hours per shift), 4 shifts per week, rotating schedule

Key Responsibilities

• Provide nursing care and support to patients in a long-term care or skilled nursing facility setting
• Follow established care plans and document patient progress accurately
• Collaborate with healthcare team members to ensure optimal patient outcomes
• Administer medications and treatments as prescribed by physicians
• Monitor patient health status and respond to patient needs in a timely manner
• Adhere to all safety and compliance standards within the facility

Required Qualifications

• Valid LVN or LPN license in the state of Pennsylvania
• Minimum of 1 year of experience in a long-term care or SNF environment preferred
• Strong patient care and documentation skills
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills

Preferred Experience / Skills

• Experience working with diverse patient populations
• Knowledge of electronic medical records (EMR) systems
• BLS certification preferred
