• Full-time, day shifts, 8-hour shifts

Key Responsibilities

• Provide comprehensive end-of-life patient care in a home health/hospice setting.
• Administer medications, including pain management and symptom control, following best practices.
• Conduct patient assessments, develop and implement care plans, and set goals with patients and their families.
• Educate patients and families on disease processes, medications, and advance care planning.
• Manage complex symptoms such as altered mental status, nausea, pressure ulcers, and cachexia.
• Collaborate with multidisciplinary teams to deliver culturally sensitive and compassionate care.

Required Qualifications

• Valid NC or Compact state RN license, in hand at the time of start.
• Minimum of 2 years of nursing experience, preferably in hospice, home health, or skilled nursing.
• Current BLS certification.
• Experience with medication administration, pain assessment, and symptom management in end-of-life care.
• Knowledge of Medicare regulations and patient/family education techniques.
• Ability to perform patient assessments and develop individualized care plans.

Preferred Experience / Skills

• Epic or other electronic health record experience, preferably in home health or hospice.
• Experience with perideath and postmortem care, spiritual needs assessments, and ethical/legal issues in end-of-life care.
• Skills in medication dose conversions, IV management, and phlebotomy.
• Certified in palliative and end-of-life care practices, including death pronouncement and counseling.
