Key Responsibilities
- Provide skilled physical therapy services to home health patients in West Portland and surrounding areas.
- Assess patient needs, develop treatment plans, and document progress using EPIC and Rover systems.
- Travel within a designated service radius, minimizing drive time and ensuring efficient patient visits.
- Maintain compliance with facility and regulatory requirements, including COVID vaccination protocols.
- Collaborate with the healthcare team to deliver quality patient care.
Required Qualifications
- Valid Oregon state licensure as a Physical Therapist.
- Minimum of 1 year of skilled home health experience.
- BCLS certification.
- Own reliable transportation.
- Ability to reliably complete Oasis documentation.
Preferred Experience / Skills
- Experience with Oasis documentation systems.
- Ability to work independently within a home health environment.
- Comfortable with travel up to approximately 60 miles per day.
