Full-time, 13-week contract, day shifts, possibly some on-call
Key Responsibilities
- Provide hospice nursing care to patients in a home or facility setting
- Manage patient cases, including assessments and care coordination
- Document patient progress and communicate with healthcare teams
- Deliver compassionate end-of-life care in accordance with hospice protocols
- May be required to provide on-call support as needed
Required Qualifications
- Valid and current Registered Nurse (RN) license in Oregon
- Experience in case management
- Strong communication and compassionate patient care skills
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Prior hospice or palliative care experience
- Comfortable with on-call responsibilities
