Key Responsibilities
- Provide high-quality physical therapy services to patients in a home care setting.
- Assess patient needs, develop treatment plans, and implement therapy interventions.
- Document patient progress and communicate effectively with the care team.
- Ensure adherence to all regulatory and safety standards.
- Maintain accurate records and complete required documentation timely.
Required Qualifications
- Valid Connecticut PT license prior to start date.
- Minimum of 1 year home care experience.
- Valid driver's license and auto insurance.
- Successful completion of fingerprinting background check and submission of necessary forms within 2 business days of offer acceptance.
Preferred Experience / Skills
- Prior experience working in home health care environments.
- Strong communication and organizational skills.
