Key Responsibilities

• Provide comprehensive physical therapy services to residents in a skilled nursing facility
• Develop and implement individualized treatment plans to improve patient outcomes
• Collaborate with a team of healthcare professionals to ensure quality care
• Support residents in achieving their rehabilitation goals and maintaining their independence
• Document patient progress and maintain accurate medical records

Required Qualifications

• Valid Physical Therapist license in South Dakota or eligibility to obtain one
• Graduate of an accredited physical therapy program
• Excellent communication skills and a team-oriented mindset
• Ability to develop personalized treatment plans based on patient needs

Preferred Experience / Skills

• Experience in skilled nursing or rehab settings preferred but new graduates are welcome
• Knowledge of current physical therapy practices and techniques
