Key Responsibilities

• Assess patients’ physical and psychosocial conditions continuously throughout their care journey.
• Develop and implement individualized patient care plans in collaboration with the healthcare team and patients’ families.
• Execute provider-directed interventions, monitor patient responses, and evaluate progress toward outcomes.
• Maintain accurate, timely documentation of patient assessments, care plans, and interventions.
• Utilize manual and electronic systems to enhance efficiency and clinical workflow.
• Serve as a liaison among nurses, physicians, and other healthcare professionals to coordinate patient care.
• Communicate care plans and significant changes effectively with the multidisciplinary team, patients, and families.
• Participate in quality improvement, risk management, and ongoing education initiatives.
• Ensure adherence to hospital policies, safety standards, and infection control protocols.

Required Qualifications

• Current State Registered Nurse license.
• Proven experience in emergency or acute care nursing.
• Knowledge of nursing procedures, equipment, medications, and triage protocols.
• Ability to perform comprehensive patient assessments and prioritize care needs.
• Strong documentation skills and familiarity with healthcare information systems.

Preferred Experience / Skills

• Experience working in a critical access hospital setting.
• Knowledge of trauma and emergency protocols.
• BLS and ACLS certifications.
