Contract | 13-Week Assignment | Full-Time | Onsite | Days and Nights Available | 8-Hour and 12-Hour Shifts | Every Other Weekend Rotation

Key Responsibilities

• Provide direct patient care within a Long-Term Care / Skilled Nursing Facility (LTC/SNF) setting

• Administer medications, treatments, and nursing interventions in accordance with the plan of care

• Monitor and document patient condition, progress, and changes, reporting to the supervising nurse or provider as appropriate

• Collaborate with interdisciplinary care teams to support resident health, safety, and comfort

• Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards

• Participate in weekend rotation coverage on an every-other-weekend basis

Required Qualifications

• Active and unrestricted LPN or LVN license valid in the state of Minnesota

• Prior experience in a Long-Term Care or Skilled Nursing Facility environment

• Ability to work both 8-hour and 12-hour shifts as assigned

• Availability to commit to a 13-week contract with a targeted start date as soon as possible

• Ability to maintain a minimum of 40 guaranteed hours per week (8–10 hour shifts) or 36 guaranteed hours per week (12-hour shifts)

Preferred Experience / Skills

• Experience working overnight or NOC shifts in a clinical care environment

• Familiarity with LTC/SNF regulatory compliance and documentation requirements

• Strong communication and teamwork skills in a multidisciplinary care setting
