Key Responsibilities
- Manage patient caseloads across multiple clinical settings including inpatient, Long Term Care, Alzheimer's, Home Health, and outpatient, from screening and evaluation through discharge planning.
- Collaborate actively as a participant in an interdisciplinary team approach to patient care.
- Communicate effectively with physicians and other rehabilitation team members regarding treatment plans and patient response to care.
- Complete all required clinical and billing documentation thoroughly and in a timely manner.
- Counsel patients and families in accordance with the degree of disability or impairment.
- Maintain compliance with established policies, procedures, quality assurance programs, safety standards, environmental guidelines, and infection control standards.
Required Qualifications
- Active Occupational Therapist licensure in the state of Colorado.
- Compliance with applicable State and Federal regulations governing occupational therapy practice.
- Ability to manage patient caseloads across diverse clinical settings independently.
- Proficiency in completing clinical documentation and billing-related records accurately and on time.
- Regular attendance and punctuality required.
Preferred Experience / Skills
- Prior experience working in Long Term Care, Alzheimer's, or Home Health settings.
- Experience participating in interdisciplinary team meetings and educational in-service programs.
