Key Responsibilities
- Provide outpatient physical therapy evaluation and treatment services to patients
- Develop and implement individualized treatment plans based on patient needs and goals
- Document patient progress and maintain accurate clinical records in accordance with facility standards
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and families on therapeutic exercises and home programs
Required Qualifications
- Active Connecticut state Physical Therapist license required prior to start
- BLS certification through the American Heart Association (AHA) required
- Minimum of 1 year of outpatient physical therapy experience
Preferred Experience / Skills
- Experience with Epic electronic health records system
