Key Responsibilities
- Provide skilled nursing care and case management for assigned hospice patients in their place of residence, including admission onto services.
- Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition and reactions to treatments or medications.
- Oversee care provided by Home Health Aides and CNAs for assigned patients and evaluate compliance with the plan of care.
- Ensure continuity of care through effective communication, case conferences, and coordination with interdisciplinary team members and community agencies.
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements.
- Participate in on-call rotation, quality improvement activities, and in-service training as assigned.
Required Qualifications
- Current Registered Nurse (RN) license in the State of Montana.
- Valid driver's license, appropriate auto insurance, and reliable transportation.
- Ability to perform clinical skills including venipuncture, catheterization, wound care, and/or parenteral therapy.
Preferred Experience / Skills
- BSN degree preferred.
- Minimum of 2 years of recent acute care, home care, or hospice nursing experience.
- Experience with HCHB (Homecare Homebase) electronic medical records system strongly preferred.
