Key Responsibilities
- Provide physical therapy services within a Long-Term Care and Skilled Nursing Facility setting
- Manage a caseload with patient ratios of approximately 1:15–25
- Evaluate, develop, and implement individualized therapy treatment plans for residents
- Document patient progress and treatment plans using facility charting software
- Collaborate with interdisciplinary care teams to support patient rehabilitation goals
Required Qualifications
- Minimum 1 year of experience in a Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) setting
- Active or pending Physical Therapist license (pending license accepted)
- Basic Cardiac Life Support (BCLS) certification required
- Must have reliable personal transportation
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Familiarity with Net Solutions charting platform
- Prior experience working in a travel or contract therapy capacity
