Contract | Days (Monday–Friday, with ability to make up missed visits on weekends) | Outpatient

Key Responsibilities

• Provide outpatient occupational therapy services, with approximately 20% of visits conducted at assisted living facilities
• Attend daily stand-up meetings at 8:30 AM, Monday through Friday
• Manage a caseload ratio of 6–7 patients
• Document patient care using Wellsky charting software
• Travel within a 50-mile radius to conduct home care and home-based visits, with an expected weekly mileage of 50–75 miles

Required Qualifications

• Active Occupational Therapist license in good standing
• Minimum of 2 years of recent licensed OT experience
• At least 1 recently completed travel assignment
• Experience with home care and home-based therapy settings
• Current BLS and First Aid certifications
• COVID-19 vaccination and booster(s) required per state guidelines (medical/religious exemptions considered on a case-by-case basis; approved exemptions require weekly testing)

Preferred Experience / Skills

• Familiarity with Wellsky or similar electronic documentation platforms
• Experience working in assisted living or similar care settings
