Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Manage a patient population that includes rehabilitation, dementia, and assisted-care residents
• Document patient care and maintain accurate records using PCC (PointClickCare) EMR
• Fulfill weekend rotation requirements (minimum every 3rd weekend, more as needed)
• Fulfill holiday rotation requirements (minimum every 3rd holiday, more as needed)
• Complete facility-specific orientation upon arrival (1–2 shifts)

Required Qualifications

• Active and unrestricted LPN or LVN license
• Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
• Ability to manage a day-shift patient ratio of 1:20
• Willingness to work 12-hour shifts
• Availability for rotating weekends and holidays as required

Preferred Experience / Skills

• Prior experience working with rehabilitation and dementia patient populations
• Familiarity with PCC (PointClickCare) electronic medical records system
• Experience working in a 50+ bed skilled nursing facility
