Key Responsibilities
- Coordinate and deliver direct physical therapy services to patients of all ages, promoting maximum independence, functionality, and safety.
- Develop, implement, and revise individualized plans of care based on assessments, client history, physician orders, and client goals, including discharge planning as appropriate.
- Perform and monitor the application of physical therapy treatments involving physical agents such as heat, water, and electricity using appropriate therapeutic equipment.
- Document all skilled and medically necessary care thoroughly, accurately, and in compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements.
- Supervise Physical Therapy Assistants and Home Health Aides in accordance with applicable Federal and State laws, regulations, and agency policy.
- Participate in staff meetings, case conferences, and quality management programs to maintain standards and support facility goals.
Required Qualifications
- Active Physical Therapist (PT) license in good standing as required by state regulations.
- Ability to perform physical therapy evaluations and develop individualized treatment plans.
- Knowledge of Medicare, State, Federal, and JCAHO documentation and compliance requirements.
- Ability to supervise PTAs and Home Health Aides in accordance with applicable laws and policies.
Preferred Experience / Skills
- Experience working with patients across a broad age range, including those with developmental deficits, physical injury, or illness.
- Prior experience in a home health or rehabilitation setting.
- Familiarity with quality improvement programs and utilization guidelines.
