Contract | Travel | 13-Week Assignment | Night Shift (6:45 PM – 7:15 AM) | Projected Start: June 15, 2026
Key Responsibilities
- Provide inpatient and home-based hospice nursing care to patients in a community/mobile setting within Buncombe County
- Serve as an RN case manager, managing a patient caseload with a 1:15 patient ratio
- Conduct comprehensive patient assessments and implement individualized pain management plans
- Provide education and emotional support to patients and their families throughout the hospice process
- Complete and maintain accurate hospice certification documentation and clinical records using Home Care Home Base EMR
- Collaborate with an interdisciplinary team including care coordinators and patient care technicians; potential for occasional backup on-call coverage
Required Qualifications
- Active Registered Nurse (RN) licensure in good standing
- Current CPR certification
- Minimum of 2 years of hospice nursing experience
- Active and valid driver's license (mobile/community-based role)
- Proficiency with electronic medical records (EMR) systems
- Strong understanding of documentation requirements for hospice certifications
Preferred Experience / Skills
- Prior experience as a hospice RN case manager with the ability to work independently
- Home health nursing experience
- Familiarity with Home Care Home Base EMR platform
