Key Responsibilities
- Provide a full range of professional nursing services to clients of all ages in a home health setting
- Perform client admissions, evaluations, and care plan development
- Collaborate with a team of homecare professionals and paraprofessionals to deliver coordinated care
- Serve clients across assigned county territories within the home health service area
Required Qualifications
- Current Vermont Registered Nurse (RN) license
- Minimum of one year of prior clinical experience
- Ability to work collaboratively within a multidisciplinary homecare team
Preferred Experience / Skills
- Prior experience in home health or hospice nursing
- Experience working with clients across a broad age range in a community-based setting
